Opioid abstinência is a clinically significant cluster of symptoms, behaviours and/or physiological features, varying in degree of severity and duration, that occurs upon cessation or reduction of uso of opioids in individuals who have developed Opioid dependência or have used opioids for a prolonged period or in large amounts. Opioid abstinência can also occur when prescribed opioids have been used in standard therapeutic doses. Presenting features of Opioid abstinência may include dysphoric mood, craving for an opioid, anxiety, nausea or vomiting, abdominal cramps, muscle aches, yawning, perspiration, hot and cold flushes, lacrimation, rhinorrhea, hypersomnia (typically in the initial phase) or insomnia, diarrhea, piloerection, and pupillary dilatation.
Opioid dependence
Opioid dependência is a transtorno of regulation of opioid uso arising from repeated or continuous uso of opioids. The characteristic feature is a strong internal drive to uso opioids, which is manifested by impaired ability to control uso, increasing priority given to uso over other activities and persistence of uso despite harm or negative consequences. These experiences are often accompanied by a subjective sensation of urge or craving to uso opioids. Physiological features of dependência may also be present, including tolerance to the effects of opioids, abstinência symptoms following cessation or reduction in uso of opioids, or repeated uso of opioids or pharmacologically similar substances to prevent or alleviate abstinência symptoms. These disturbances are caused by the known pharmacological effects of opioids and their intensity is closely related to the amount of opioids consumed. They are time-limited and abate as opioids are cleared from the body. Presenting features may include somnolence, stupor, mood changes (e.g. euphoria followed by apathy and dysphoria), psychomotor retardation, impaired judgment, respiratory depression, slurred speech, and impairment of memory and attention. In grave intoxicação coma may ensue. A characteristic physical sign is pupillary constriction but this sign may be absent when intoxicação is due to synthetic opioids. grave opioid intoxicação can lead to death due to respiratory depression.

Oppositional defiant transtorno is a persistent padrão (e.g., 6 months or more) of markedly defiant, disobedient, provocative or spiteful behaviour that occurs more frequently than is typically observed in individuals of comparable age and developmental level and that is not restricted to interaction com siblings. Chronic irritability and anger are characteristic of the individual’s functioning nearly every day, are observable across multiple settings or domains of functioning (e.g., home, school, social relationships), and are not restricted to the individual’s relationship com his/her parents or guardians. The padrão of chronic irritability and anger is not limited to occasional episodes (e.g., developmentally typical irritability) or discrete periods (e.g., irritable mood in the context of manic or depressive episodes).

Meets all definitional requirements for oppositional defiant transtorno. This form of oppositional defiant transtorno is not characterised by prevailing, persistent, angry or irritable mood, but does feature headstrong, argumentative, and defiant behaviour.
Oppositional defiant disorder without chronic irritability-anger with limited prosocial emotions
All definitional requirements for oppositional defiant transtorno sem chronic irritability-anger are met. In addition, the individual exhibits characteristics that are sometimes referred to as ‘callous and unemotional’. These characteristics include a lack of empathy or sensitivity to the feelings of others and a lack of concern for others’ distress; a lack of remorse, shame or guilt over their own behaviour (unless prompted by being apprehended), a relative indifference to the probability of punishment; a lack of concern over poor performance in school or work; and limited expression of emotions, particularly positive or loving feelings toward others, or only doing so in ways that seem shallow, insincere, or instrumental.
